Sister of Mercy is a mysterious six-armed being who is part of the Scarlet Church. She is capable of floating in the air and telekinesis which makes her a threat to Aidan's attempt at taking down the cult.
Just like Colin Harlowe, she views herself a goddess and a choice of the : Extradimensional beings who chase extreme sensation and put humans through endless experiments.
Pictures and notes around the Scarlet Church suggest that the Sister of Mercy and Harlowe have a relationship. Her dialogue on the pains of getting "top surgery" also suggest she was originally a man.

Sister of Mercy's First Appearance
You first meet Sister of Mercy while chasing after Harlowe in your return to the Scarlet Church. The Sister of Mercy is in the room behind a diamond-shaped puzzle that requires turning four different pieces to show a woman sleeping.
The Sister of Mercy immediately grabs and lifts you, Aidan Lynch, in the air and throws away the Genesis Configuration. Without the Genesis Configuration, there is no way of defeating the Sister of Mercy and you are forced to sneak away to the Labyrinth to get back the Genesis Configuration.
