You reach Bruno's office after you change into the Cult's Clothes and sneak past the guard. Go through the small office, return through the hallway, and enter Bruno's office.
How to Solve the Pinhead Skull Puzzle
[1] Go to Bruno's Desk
Enter Bruno's office and go to his desk. Pick up the memo, then investigate the Pinhead skull.
[2] Push Down Every Pin
Interact with all the pins on the skull to push them down. Once every pin is down, you get the Tongue Key.
[3] Use the Tongue Key on the Open-Mouth Door
Take the Tongue Key to the door with the open mouth and use it to unlock the door. Check the bathroom in Bruno's office first, since it has a Cloth on the shelf.
