You reach this puzzle after the worship hall. Once you kill the cultist who walks in on the bodies, enter the office at the end of the hallway.
How to Solve the Leviathan Tip Puzzle
[1] Take the Leviathan Tip From the Desk.
In the office, go to the computer on the left. The Leviathan Tip is on the desk next to it, along with a Sausage you can eat to heal.
[2] Use the Tip on the Strange Object.
Return to the church room and go to the opposite side to return to the hallway. Use the Leviathan Tip on the strange object in the middle.
[3] Rotate the Pieces Until the Lines Connect.
Rotate the tips and the middle until all the lines connect to each other.
[4] Collect the S&M Duckie.
Once the lines connect, you receive the S&M Duckie. Keep it, since you use it on a door in the office area next.
