While you're picking up Nightingale's Heart in the Cauldron Lake General Store, a Taken will smash through the wall, opening up a back room you couldn't reach before. Head inside, and you'll spot the Sawed-Off Shotgun locked away in a display cabinet. Enter 739 to unlock it and claim the weapon.
