The 5th nursery rhyme at Cauldron Lake will appear after you complete all 16 other nursery rhymes. When you clear the 16th puzzle, a trunk containing a doll of your father will appear, so collect the doll before heading there.

Charms are equipment with effects that give you an advantage in exploration and combat, and you can equip up to 3 types. Charms can be found near the nursery rhyme puzzle or beyond black puddles and footprints, so explore the area after solving the puzzle to obtain them.
A Collectible and One of the Trophy Conditions
Nursery rhymes are one of the Collectibles. When you solve all the nursery rhyme puzzles, you will obtain the trophy "Reality Changes," so if you are aiming for complete, actively take on the challenge.
